FORM PURPOSE
Use this form to request cancellation of a Primary Membership and all associated members. To add or remove associated members without canceling the Primary Membership, please contact the facility directly.

MEMBERSHIP TERMINATION POLICY
Membership termination requests must be submitted by the 20th of the month to take effect on the 1st of the following month. All dues must be paid for the cancellation to be valid.

Requests received after the 20th will take effect on the 1st of the next month, and dues for the upcoming month will still be owed. Example: A request received on February 21 ends the membership on April 1.

Per City of Calabasas policy, memberships are non-refundable and cannot be prorated.
